Liability Waiver, 2024-25 Season

As legal guardian of a minor, who is a member under my account, I consent by signing below and entrust the care of my child to the supervision of the coach(es) and the chaperones of the BLAST Swim Club. I assume responsibility for the costs of any damage caused by my child at the pool or elsewhere. I agree to pay all costs incurred by any disciplinary action imposed on my child. In the event of illness or accident, I authorize the coach(es) or chaperone to sign all the forms required for the urgent medical care necessary, if the persons responsible could not be contacted within a reasonable time.

Exonération de responsabilité, Saison 2024-25

Comme tuteur(trice) légal(e) de mineur inscrit comme membre sous mon compte, je consente, par la présente en mon nom à confier la charge de mon enfant à la surveillance de (des) l’entraineur(s) et des chaperons du club de natation BLAST. J’assume la responsabilité des couts pour tout dommage causé par mon enfant à la piscine ou ailleurs. J’accepte de payer tous les frais encourus par une action disciplinaire imposée à mon enfant. En cas de maladie ou accident, j’autorise l’(les) entraineur(s) ou chaperon à signer toutes les formulaires requis pour les soins médicaux urgents, si les personnes responsables n’ont pu être contactées dans un délai raisonnable.
